The Diagnostic Value Of Specific Lge For Food Allergen, Histamine Release Test And Skin Prick Test In Children With Food Allergy

RationaleDouble-blind placebo-controlled food challenges (DBPCFCs) are considered as the gold standard for the diagnosis of food allergy. But to reduce the risk of oral food challenges (OFCs), evaluation of other tests is needed. We examined the diagnostic value of specific IgE antibody for food allergen, histamine release test and skin prick test in children with food allergy.MethodsSpecific IgE antibody for food, histamine release test and skin prick test were performed in 119 patients (range, 2 y 1 mo to 17 y 3 mo, mean 7 y 1 mo) diagnosed as having food allergy (egg, milk, wheat and soy) by DBPCFCs. We evaluated specific IgE antibody for food and histamine release test in egg oral desensitization patients also.ResultsThe rate of the positive result in DBPCFCs is 80/97 for egg, 47/101 for milk, 20/108 for wheat and 5/115 for soy. The sensitivity/specificity of specific IgE for egg is 98.7%/50.0%, 97.8%/69.6% for milk, 100%/24.0% for wheat and 80.0%/18.8% for soy. That of histamine release test for egg is 83.9%/100%, 85.7%/92.5% for milk, 80.0%/85.0% for wheat and 50.0%/82.7% for soy. That of skin prick test for egg is 100%/44.4%,100%/46.2% for milk, 100%/33.3% for wheat and 75.0%/45.5% for soy. And that of the combination of them for egg is 100%/100%. But, this result does not apply to the egg oral desensitization patients.ConclusionsThe sensitivity of specific IgE antibody for food allergen and skin prick test is high and the specificity of histamine release test is high. The combination of them is useful for diagnosis of egg allergy in children except the egg oral desensitization patients.
