Le PSA vu par le clinicien

L'intérêt du dosage du PSA en pathologie prostatique (en particulier pour le suivi du cancer de la prostate) est bien connu. Cependant, le dosage du PSA total se caractérisait par une grande variabilité inter-technique mise en évidence sur les sérums de patients et sur les sérums de contrôle. L'analyse de résultats, obtenus dans le cadre du contrôle de qualité ponctuel des « Immunodosages avec marquer non radioactif å ProBioQual, montre une diminution de la variabilité totale entre 1997 et 1999. En effet, les CV Tr toutes techniques étaient > 30 % (de 31,0 à 50,6 %) en 1997 et < 20 % (de 9,0 à 19,1 %) en 1999 pour des concentrations moyennes comprises entre 1,0 et 40 μg L−1. La diminution de la variabilité totale est expliquée par la modification de certaines trousses (calibration par rapport à l'étalon Stanford 90 : 10 et dosage équimolaire du PSA libre et du PSA lié à l'alpha 1 antichymotrypsine) et par la composition des sérums de contrôle.Evaluation of assay results for total PSA; Data obtained within the framework of an external quality assay survey. The measurement of PSA (prostate-specific antigen) is widely used in the monitoring of patients with prostate cancer. However, the estimation of the concentrations was caracterized by large differences between kits in clinical specimen and control samples. The analysis of the results, obtained from an external quality assessment scheme for non isotopic immunoassays, shows a decrease of the total variability between 1997 and 1999. Indeed, the CV Tr, all methods mixed, were > 30 % (from 31,0 to 50,6 %) in 1997 and were < 20 % (from 9,0 to 19,1 %) in 1999 for the specimens analysed in a range from 1,0 to 40 μg L−1. The decrease of the total variability is explained by the modification of some kits (use of the Stanford calibrator 90 : 10 and equimolar response to free PSA and PSA complexed to alpha 1 antichymotrypsin) and also by the composition of the commercially control samples.
