I attended Columbia University College of Physicians and Surgeons for medical school, where I began my research career with Dr. Hiroo Takayama analyzing single center outcomes of temporary mechanical circulatory support in cardiogenic shock. Staying at Columbia for internal medicine residency, I began to focus my efforts on clinical research in durable left ventricular assist device therapies as bridge to heart transplantation. Working with Dr. Veli Topkara and Dr. Maryjane Farr, I was the first author of multiple publications highlighting the nationwide variability in the use of this listing strategy as well as implications of ventricular assist device support on early post-transplant outcomes. I began to focus, particularly, on the risk of primary graft dysfunction in patients bridged to transplant with these devices, and after publishing our single center results, I worked with colleagues from Stanford University to develop the International Consortium on Primary Graft Dysfunction: an international, observational cohort of over 2500 heart transplant recipients from the United State and Europe. Leveraging this large and well-phenotyped population, we have been able to, for the first time, define clinical risk factors for primary graft dysfunction in the contemporary era and demonstrate that older risk scoring systems no long adequately capture donor and recipient risk.
The move to Duke for cardiology fellowship was a natural one, given the strong reputation of Duke Cardiology in translational and basic cardiovascular research. Moreover, the Duke fellowship offered unparalleled research opportunities for its trainees by providing a minimum of two years of funded research under its NIH T32 training grants. I was very fortunate to meet with Dr. Svati Shah early on in my fellowship and developed a plan to spend my dedicated research time in her lab learning more about multiomic profiling and how it may be applied in the heart transplant space. During my dedicated research time and given my clinical interests in primary graft dysfunction, I designed and executed a project to define pre-transplant biomarkers of primary graft dysfunction in a large, well-phenotyped cohort of heart transplant recipients. We found a novel biomarker, CLEC4C, to be increased in the circulation of recipients who would go on to develop moderate to severe primary graft dysfunction and validated these signals in a separate cohort, even after adjustment for other important clinical covariates. This work has been published in the Journal of Heart and Lung Transplantation. We submitted additional abstracts leveraging mass spectroscopy based proteomics for biomarkers of graft dysfunction in static cold storage preservation solution and also highlighted circulating pre-transplant proteins that were associated with post-transplant renal failure. Finally, we recently received funding to perform single cell RNA sequencing on myocardial tissue from heart explanted for severe cardiac allograft vasculopathy to examine the transcriptomic landscape of chronic allograft rejection. During my time at Duke I was fortunate to receive an NIH Loan Repayment Award and Renewal as well as a Mandel Family Foundation Award, a Translating Duke Health grant, and a Duke Transplant Center Pilot Award. I was also the recipient of the Joseph Greenfield Research Scholar Award which provided additional salary support and a research stipend. During my research time at Duke I completed a Masters in Biostatistics through the Duke University School of Medicine.
The experiences above have solidified my desire to pursue a career as an academic physician-scientist in advanced heart failure cardiology. I have recently transitioned to the University of Texas Southwestern as an Advanced Heart Failure and Transplant fellow, where I am continuing my work leveraging omics technologies to better understand and characterize post-transplant outcomes.
