I am Full professor of Physiology at the University of Murcia and I focused my investigations in the study of regulatory mechanisms of arterial blood pressure.
We have done different works focused on the study of the participation of the system renin-angiotensin and its relation to ovarian hormones in the pathophysiology of different diseases, such as ovarian hyperstimulation syndrome, hypertension and menopause and more recently of preeclampsia.
In summary, our studies show that estradiol produces an improvement in endothelial function and blood pressure in ovariectomized rats with a healthy endothelium, while in hypertensive and elderly rats, replacement therapy with estradiol does not improve the evolution of hypertension or the vascular remodelling that it carries. The metabolite of estradiol 2-methoxyestradiol increases the bioavailability of NO producing a vasodilator effect in microvessels of the mesenteric bed and improves the evolution of hypertension and associated coronary remodelling in female ovariectomized rats and male rats (spontaneously hypertensive rats). The studies in pregnant rats show that the pharmacological inhibition of COMT (enzyme that catalyzes the synthesis of 2-methoxyestradiol) causes an increase in blood pressure and endothelial dysfunction, similar to human preeclampsia. Presence in the fetus of the Val158Met polymorphism in the COMT gene increases the risk of preeclampsia in pregnant women, a fact that coincides with a lower expression and enzymatic activity of the Placental COMT. Women with preeclampsia have lower plasma levels of 2-methoxyestradiol than the pregnant controls, there being a significant correlation between such levels and anti-angiogenic factors as sflt-1, higher blood pressures, and other indicators of severity of preeclampsia. All those results suggest an important role of estrogens and some metabolites on the control or vascular endothelium and blood pressure.
At the present, we are participant in a clinical multicentric study to investigate de role of pravastatin in the prevention of late onset of preeclampsia. As a part of the global study we are interested in the study of the pravastatin effects on vascular endothelial function and its relationship to prevention of preeclampsia.
